When will I need a D4 medical examination

You will need D4 medical examination for HGV/LGV drivers:

  • When applying for or renewing a Group 2 (HGV/LGV) driving licence.
  • Every 5 years from age 45 and annually from age 65.
  • At DVLA's request and if you have certain medical conditions.

The D4 Medical Form must be completed by an occupational health (OH) doctor or your GP.

Complete the grey sections of the D4 Form

Bring your prescriptions and driving glasses prescriptions if applicable.

On completion, the doctor will give you the completed D4 Form to be posted to DVLA.

What do I need to bring to my D4 HGV/LGV medical exam in Bristol?

Two forms of identification (e.g., driver's license and a recent bill)

Completed D4 Medical Examination Form. Complete the grey sections of the D4 Form.

Bring your prescriptions and driving glasses prescriptions if applicable.

What happens if I have a medical condition? Do I still need a D4 Medical?

Yes, you will still need a D4 Medical.

You must tell DVLA if you have a ‘notifiable condition’ using the GOV.UK website: diabetes (or taking insulin), syncope (fainting), heart conditions (including atrial fibrillation and pacemakers),  sleep apnoea, epilepsy, stroke, glaucoma.

The doctor will assess your fitness to drive according to DVLA guidelines and complete the D4 form. If your condition requires further information, DVLA may contact your GP or specialist.
